I hereby voluntarily allow Minors/participants to participate in playing on the bounce house/inflatable provided by the T&SInflatables.I hereby confirm that the Minors/Participants are in good physical condition and do not suffer from any disabilities or physical conditions that places me or others at risk or otherwise should prohibit their participation in this event. I am aware that the use of the bounce house/inflatable involves inherent risks, including but not limited to injury from falling, colliding, or improper use, and I voluntarily assume all risks related to my participation. In consideration of being permitted to use the bounce house/inflatables for minors, I hereby release, waive, discharge, and covenant not to sue the Organization, its owners, employees, agents, and representatives from any and all claims, liabilities, damages, or expenses, including but not limited to personal injury or property damage, arising from or related to my use of the bounce house, except where such injury or damage is caused by the gross negligence or willful misconduct of the Organization/Business/Participant Leader. TSInflatables has a no refundable deposit. We are not liable to refund for an event due to unreliable circumstances not associated with TSInflatables.

In the event of an injury or medical emergency, I authorize the Organization/Business/Participant Leader

care. I understand in the event of any property damage, the item will automatically have a minimum charge of $200 or total item replacement depending on the damage caused. I have read this waiver and fully understand its contents. I acknowledge that by signing this waiver, I am giving up substantial legal rights, including the right to sue the Organization, T&S INFLATABLES.
